ABSTRACT

In order to comprehensively evaluate the quality of Viticis Fructus, this study established HPLC fingerprints and evaluated the quality of 24 batches of Viticis Fructus samples from different species by similarity evaluation and multivariate statistical analysis(PCA, HCA, PLS-DA). On this basis, an HPLC method was established to compare the content differences of the main components, including casticin, agnuside, homoorientin, and p-hydroxybenzoic acid. The analysis was performed on the chromatographic column(Waters Symmetry C_(18)) with a gradient mobile phase of acetonitrile(A)-0.05% phosphoric acid solution(B) at the flow rate of 1 mL·min~(-1) and detection wavelength of 258 nm. The column temperature was 30 ℃ and the injection volume was 10 μL. The HPLC fingerprint of 24 batches of Viticis Fructus samples was established with 21 common peaks, and nine peaks were identified. Similarity analysis was carried out based on chromatographic data of 24 batches of chromatographic data of Viticis Fructus, and the results showed that except for DYMJ-16, the similarity of Vitex trifolia var. simplicifolia was ≥0.900, while that of V. trifolia was ≤0.864. In addition, the similarity analysis of two different species showed that the similarity of 16 batches of V. trifolia var. simplicifolia was 0.894-0.997 and that of the eight batches of V. trifolia was between 0.990 and 0.997. The results showed that the similarity of fingerprints of these two species was different, but the similarity between the same species was good. The results of the three multivariate statistical analyses were consistent, which could distinguish the two different species. The VIP analysis results of PLS-DA showed that casticin and agnuside contributed the most to the distinction. The content determination results showed that there was no significant difference in the content of homoorientin and p-hydroxybenzoic acid in Viticis Fructus from different species, but the content of casticin and agnuside was significantly different in different species(P<0.01). The content of casticin was higher in V. trifolia var. simplicifolia, while agnuside was higher in V. trifolia. The findings of this study show that there are differences in fingerprint similarity and component content of Viticis Fructus from different species, which can provide references for the in-depth study of the quality and clinical application of Viticis Fructus.

ABSTRACT

Background@#Dysfunction of vascular endothelial cells (ECs) plays a central role in the pathogenesis of cardiovascular complications in diabetes. SWI/SNF-related matrix-associated actin-dependent regulator of chromatin subfamily A member 5 (SMARCA5) is a key regulator of chromatin structure and DNA repair, but its role in ECs remains surprisingly unexplored. The current study was designed to elucidate the regulated expression and function of SMARCA5 in diabetic ECs. @*Methods@#SMARCA5 expression was evaluated in ECs from diabetic mouse and human circulating CD34+ cells using quantitative reverse transcription polymerase chain reaction and Western blot. Effects of SMARCA5 manipulation on ECs function were evaluated using cell migration, in vitro tube formation and in vivo wound healing assays. Interaction among oxidative stress, SMARCA5 and transcriptional reprogramming was elucidated using luciferase reporter assay, electrophoretic mobility shift assay and chromatin immunoprecipitation. @*Results@#Endothelial SMARCA5 expression was significantly decreased in diabetic rodents and humans. Hyperglycemia-suppressed SMARCA5 impaired EC migration and tube formation in vitro, and blunted vasculogenesis in vivo. Contrarily, overexpression of SMARCA5 in situ by a SMARCA5 adenovirus-incorporated hydrogel effectively promoted the rate of wound healing in a dorsal skin punch injury model of diabetic mice. Mechanistically, hyperglycemia-elicited oxidative stress suppressed SMARCA5 transactivation in a signal transducer and activator of transcription 3 (STAT3)-dependent manner. Moreover, SMARCA5 maintained transcriptional homeostasis of several pro-angiogenic factors through both direct and indirect chromatin-remodeling mechanisms. In contrast, depletion of SMARCA5 disrupted transcriptional homeostasis to render ECs unresponsive to established angiogenic factors, which ultimately resulted in endothelial dysfunction in diabetes. @*Conclusion@#Suppression of endothelial SMARCA5 contributes to, at least in part, multiple aspects of endothelial dysfunction, which may thereby exacerbate cardiovascular complications in diabetes.

ABSTRACT

Objective:To explore the prevalence rate of restless leg syndrome (RLS) and its related influencing factors in patients with maintenance hemodialysis (MHD) in Anhui province.Methods:It was a cross-sectional study, including regular MHD patients in blood purification centers of 27 hospitals in southern, central and northern Anhui province from January to March 2020. The investigation included general demographic characteristics, primary diseases, complications, dialysis-related conditions and drug use. According to the presence or absence of RLS, the patients were divided into RLS group and non-RLS group, and the differences of clinical data between the two groups were compared. Logistic regression analysis method was used to analyze the influencing factors of RLS.Results:The study included 3 025 MHD patients, aged (54.8±12.8) years old, with 1 819 males (60.1%) and 1 206 females (39.9%). The dialysis age was (5.5±3.8) years. The prevalence rates of RLS in MHD patients were 8.8% (265/3 025), of which 13.7% (77/561), 7.7% (88/1 145) and 7.6% (100/1 319) in southern, central and northern Anhui province, respectively. The prevalence rates of RLS in MHD patients in secondary and tertiary hospitals were 9.5% (55/577) and 8.6% (210/2 448), respectively. Proportion of hypertension, dialysis age, intact parathyroid hormone, alkaline phosphatase, 25(OH)D3, and proportions of secondary hyperparathyroidism, erythropoietin use, iron supplements, active vitamin D use, calcimimetic use, dialysis blood flow <250 ml/min and low-flux dialyzers in RLS group were higher than those in non-RLS group, while hemoglobin level and proportions of non-calcium-phosphorus binders and high-flux dialyzers were lower than those in non-RLS group (all P<0.05). Multivariate logistic regression analysis results showed that long dialysis age ( OR=1.188, 95% CI 1.031-1.369, P=0.017) and high alkaline phosphatase ( OR=1.007, 95% CI 1.001-1.013, P=0.047) were correlated with a higher risk of RLS in MHD patients. Conclusions:The prevalence rate of RLS in MHD patients in Anhui province is 8.8%. The prevalence rates of RLS in MHD patients in southern, central and northern Anhui province are 13.7%, 7.7% and 7.6%, respectively. The prevalence rate of RLS in secondary and tertiary hospitals are 9.5% and 8.6%, respectively. Long dialysis age and high alkaline phosphatase are associated with a high risk of RLS in MHD patients.

Acute kidney injury (AKI) caused by oxalate nephropathy is one of the reasons which is easy to be ignored in clinic. The paper reported two cases of secondary oxalate nephropathy complicated by AKI confirmed by renal biopsy, and renal function recovered in patients with active treatment. The literatures on oxalate nephropathy were reviewed to improve clinicians' understanding of oxalate nephropathy.

ABSTRACT

OBJECTIVE@#Exposure to high intensity, low frequency noise (HI-LFN) causes vibroacoustic disease (VAD), with memory deficit as a primary non-auditory symptomatic effect of VAD. However, the underlying mechanism of the memory deficit is unknown. This study aimed to characterize potential mechanisms involving morphological changes of neurons and nerve fibers in the hippocampus, after exposure to HI-LFN.@*METHODS@#Adult wild-type and transient receptor potential vanilloid subtype 4 knockout (TRPV4-/-) mice were used for construction of the HI-LFN injury model. The new object recognition task and the Morris water maze test were used to measure the memory of these animals. Hemoxylin and eosin and immunofluorescence staining were used to examine morphological changes of the hippocampus after exposure to HI-LFN.@*RESULTS@#The expression of TRPV4 was significantly upregulated in the hippocampus after HI-LFN exposure. Furthermore, memory deficits correlated with lower densities of neurons and neurofilament-positive nerve fibers in the cornu ammonis 1 (CA1) and dentate gyrus (DG) hippocampal areas in wild-type mice. However, TRPV4-/- mice showed better performance in memory tests and more integrated neurofilament-positive nerve fibers in the CA1 and DG areas after HI-LFN exposure.@*CONCLUSION@#TRPV4 up-regulation induced neurofilament positive nerve fiber injury in the hippocampus, which was a possible mechanism for memory impairment and cognitive decline resulting from HI-LFN exposure. Together, these results identified a promising therapeutic target for treating cognitive dysfunction in VAD patients.

ABSTRACT Introduction: The physical education curriculum aims to improve the level of students, benefiting the development of physical and mental health. Currently, the original physical education curriculum does not meet the specific requirements of today's students and needs to undergo a new stage of restructuring in its development. Objective: Analyze the impacts of gymnastics training on the physical fitness of female college students. Methods: We selected 86 female college students who chose the gymnastics elective course. The volunteers were divided into an experimental (n=43) and a control group (n=43). While the control performed basic training, the experimental group performed aerobics training according to an established protocol. The experiment, 60 minutes daily, was performed twice a week for 18 weeks. Data related to body composition and functional physical performance were collected before and after the intervention, and a comparative analysis was performed with statistical software. Results: The experimental group showed considerable gains; body fat ratio decreased from 27.99 to 26.25; vital capacity increased by 15.13%, and maximum strength performance in one minute increased by 15.02%. Conclusion: According to the results presented, gymnastics training significantly improved female college students' physical fitness. Its constant exercise also has the potential to improve the muscle tone of the pelvic girdle and abdomen. Level of evidence II; Therapeutic studies - investigation of treatment results.


RESUMO Introdução: O objetivo do currículo de educação física é melhorar o nível dos alunos, beneficiando o desenvolvimento da saúde física e mental. Atualmente, o currículo original de educação física não atende às exigências específicas das alunas atuais, necessitando passar por uma nova etapa de reestruturação em seu desenvolvimento. Objetivo: Analisar os impactos do treinamento de ginástica sobre a aptidão física das estudantes universitárias. Métodos: Selecionou-se 86 estudantes universitárias que elegeram o curso optativo de ginástica. As voluntárias foram divididas em grupo experimental (n=43) e controle (n=43). Enquanto o controle realizava o treinamento básico, o grupo experimental realizou um treinamento de aeróbica segundo um protocolo estabelecido. O experimento de 60 minutos diários, foi realizado duas vezes por semana durante 18 semanas. Dados relacionados à composição corporal e desempenho físico funcional foram coletados antes e após a intervenção e uma análise comparativa foi realizada com software estatístico. Resultados: O grupo experimental apresentou ganhos consideráveis, a taxa de gordura corporal diminuiu de 27.99 para 26.25; a capacidade vital aumentou 15.13%; e o desempenho de força máxima em um minuto aumentou 15.02%. Conclusão: De acordo com os resultados apresentados, o treinamento de ginástica apresentou efeitos significativos na melhoria da aptidão física das estudantes universitárias. O seu exercício constante também tem o potencial de melhorar o tônus muscular da cintura pélvica e do abdômen. Resumen Introducción: El objetivo del currículo de educación física es mejorar el nivel de los alumnos, beneficiando el desarrollo de la salud física y mental. En la actualidad, el plan de estudios original de educación física no satisface los requisitos específicos de los alumnos actuales, por lo que necesita pasar por una nueva etapa de reestructuración en su desarrollo. Objetivo: Analizar las repercusiones del entrenamiento gimnástico en la aptitud física de estudiantes universitarias. Métodos: Se seleccionaron 86 alumnas universitarias que eligieron la asignatura optativa de gimnasia. Los voluntarios se dividieron en grupo experimental (n=43) y grupo de control (n=43). Mientras el grupo de control realizaba entrenamiento básico, el grupo experimental realizaba entrenamiento aeróbico según un protocolo establecido. El experimento, de 60 minutos diarios, se realizó dos veces por semana durante 18 semanas. Se recogieron datos relacionados con la composición corporal y el rendimiento físico funcional antes y después de la intervención y se realizó un análisis comparativo con programas informáticos estadísticos. Resultados: El grupo experimental mostró ganancias considerables, la proporción de grasa corporal disminuyó de 27,99 a 26,25; la capacidad vital aumentó un 15,13%; y el rendimiento de fuerza máxima en un minuto aumentó un 15,02%. Conclusión: Según los resultados presentados, el entrenamiento gimnástico mostró efectos significativos en la mejora de la aptitud física de las estudiantes universitarias. Su ejercicio constante también tiene el potencial de mejorar el tono muscular de la cintura pélvica y el abdomen. Objective:To evaluate the efficacy of visual laryngoscope for tracheal intubation for resuscitation in the premature infants born within 32 weeks of gestation.Methods:A total of 84 premature infants who were born < 32 weeks of gestation in the Third Affiliated Hospital of Zhengzhou University and required tracheal intubation after birth from January 2019 to December 2021 were enrolled and divided into direct laryngoscope group and visual laryngoscope group according to the random number table method, with 42 cases in each group.The glottis exposure, intubation time, successful tracheal intubation at first attempt and complications related to intubation were recorded in the two groups.Results:Compared with direct laryngoscope group, the time of glottis exposure and tracheal intubation was significantly shortened, and the success rate of endotracheal intubation at first attempt was increased in visual laryngoscope group ( P<0.05).There was no significant difference in the glottis exposure during laryngoscope-assisted intubation and incidence of complications related to tracheal intubation between the two groups ( P>0.05). Conclusions:Visual laryngoscope provides better efficacy than direct laryngoscope when used for tracheal intubation for resuscitation in the premature infants born within 32 weeks of gestation.

ABSTRACT

Objective: To analyze the migration of the HIV/AIDS cases and related factors in Liangshan Yi autonomous prefecture (Liangshan). Methods: According to HIV/AIDS Comprehensive Response Information Management System of China Information System for Disease Control and Prevention, a total of 28 772 HIV/AIDS cases who had follow-up records in Liangshan in 2020 were included in the survey. The migration of the HIV/AIDS cases was described and the related factors were analyzed using multiple logistic regression models, and the migration destinations of the HIV/AIDS cases were mapped. Results: Among the 28 772 HIV/AIDS cases, 20.89% (6 010/28 772) had migration in 2020. Multivariate logistic regression analysis showed that among the HIV/AIDS cases, the migration related factors included being aged 15-24 years (compared with being aged 0-14 years, OR=2.74, 95%CI:2.04-3.69) and ethnic group (compared with Han ethnic group, OR=2.44, 95%CI:2.19-2.72), having education level of junior high school (compared with having education level of primary school or below, OR=1.25, 95%CI:1.14-1.38), being unmarried (compared with being married, OR=1.29, 95%CI:1.20-1.39), being engaged in business services (compared with being engaged in farming, OR=1.96, 95%CI:1.31-2.92), receiving antiviral treatment <1 year (compared with receiving antiviral treatment >3 years, OR=1.42, 95%CI:1.26-1.61), having recent CD4+T lymphocytes (CD4) counts >500 cells/μl (compared with having recent CD4 counts <200 cells/μl, OR=1.15, 95%CI:1.03-1.29). The geographical distribution maps showed that among all cities in Sichuan, Xichang (13.26%, 797/6 010) and Chengdu (10.12%,608/6 010) were the main migration destinations of the HIV/AIDS cases, and the provinces outside Sichuan where the HIV/AIDS cases would like to migrate to were mainly Guangdong (18.19%, 1 093/6 010) and Zhejiang provinces (7.67%, 461/6 010) in 2020. The HIV/AIDS cases who migrated where Liangshan, within Sichuan province, and to other provinces accounted for 27.67% (1 663/6 010), 15.34% (922/6 010) and 56.99% (3 425/6 010), respectively. Conclusions: More attention should be paid to the mobility characteristics and the classification management of HIV/AIDS cases according to their characteristics in Liangshan. Timely access to information on changes in the place of work and residence of HIV/AIDS cases should be warranted when they have migration. Good referrals and management for mobility of HIV/AIDS cases in different places should be made to reduce loss to follow-up and improving interventions.

ABSTRACT

Objective:To explore the effect of the 3D printed model assisted network teaching of neurosurgery.Methods:Seventy-six probation students were divided into: 3D printed model assisted online course teaching group (test group) and traditional teaching group (control group). After 1-month online teaching, quantitative assessment and satisfaction survey were carried out. SPSS 17.0 was used for statistical analysis of variance.Results:The department graduation test (theory and operation) scores of students in the test group were (86.7±7.4) points, which was significantly higher than those in the control group (78.2±8.2) points ( t=5.56, P<0.01). The teaching satisfaction survey showed that the test group had significantly higher scores in self-evaluation of spatial imagination ( t=3.81, P<0.01), deepening understanding of neuroanatomy ( t=5.29, P<0.01), and increasing interest in clinical learning ( t=5.12, P<0.01) than those of the control group. Conclusion:Compared with the conventional online teaching methods, 3D printed model assisted online teaching helps to improve teaching quality and students' satisfaction.

ABSTRACT

Objective:To probe the security and effectiveness of laparoscopic mustard surgery in elderly patients with advanced gastric cancer and that of open label laparoscopic mustard surgery by Meta-analysis.Methods:From database creation to June 2022, PubMed, Cochrane Library, Web of Science, Embase, CNKI, Wanfang database, VIP database and Chinese Biomedical Literature (CBM) database were searched. RevMan5.3 software was used to make forest maps for Meta-analysis, and funnel plot was used to evaluate publication bias, and Egger was used to detect whether there was publication bias. The operation time, intraoperative blood loss, number of lymph nodes remove, time to first flatus, time to first oral diet, time to ambulation, length of hospital stay, and overall postoperative complications rate, incidence of postoperative wound infection, incidence of pulmonary infection, incidence of postoperative intestinal obstruction and incidence of anastomotic leakage were included and analyzed.Results:A total of 11 contributions were submitted, including 2 randomized controlled trials and 9 retrospective studies. The total number of patients included in this study was 1081, including 519 in the abdominal group and 562 in the open group. Results showed that the intraperitoneal group had less bleeding compared to open surgery ( MD=-104.21, 95% CI: -132.54--75.89, P<0.00001), earlier first flatus ( MD=-1.90, 95% CI: -2.19--1.49, P<0.00001), the first time to oral diet was earlier ( MD=-1.92, 95% CI: -2.48--1.35, P<0.00001), the time of first landing after operation was earlier ( MD=-2.89, 95% CI: -3.32--2.45, P<0.00001), the length of hospital stay was shorten ( MD=-6.08, 95% CI: -7.20--4.97, P<0.00001), a lower overall postoperative complication rate ( OR=0.40, 95% CI: 0.29-0.56, P<0.00001), the incidence of postoperative incision infection was lower ( OR= 0.33, 95% CI: 0.17-0.67, P=0.002), the incidence of pulmonary infection was lower ( OR= 0.39, 95% CI: 0.22-0.69, P=0.001), the operation time was longer ( MD=30.35, 95% CI: 14.34-46.35, P=0.0002), and the differences were statistically significant ( P<0.05). There are no statistical significance in the number of lymph nodes remove( MD=-0.52, 95% CI: -1.76-0.73, P=0.41), the incidence of postoperative intestinal obstruction ( OR= 0.38; 95% CI: 0.13-1.13; P=0.08) and the incidence of anastomotic leakage ( OR= 0.41, 95% CI: 0.16-1.06, P=0.07) between two groups. Conclusions:Compared with open radical gastrectomy, laparoscopic radical gastrectomy is safe and feasible for elderly patients with advanced gastric cancer as well, and even better than open radical gastrectomy in curative effect and radical treatment.

ABSTRACT

@#Objective    To investigate the best anatomical classification, surgical timing, procedure and clinical outcomes of congenital vascular ring. Methods    The clinical data of 58 patients who underwent congenital vascular ring surgery in Pediatric Surgery Center, Fuwai Hospital between 2014 and 2019 were retrospectively analyzed. There were 32 (55.2%) males and 26 (44.8%) females with a median age of 16.5 (2-73) months. Preoperative symptoms, imaging examinations, anatomical classifications, surgical procedures and postoperative recovery were assessed. Results    There were 20 (34.5%) patients of double aortic arch, 22 (37.9%) patients of right aortic arch with left arterial duct or ligament, 15 (25.9%) patients of left aortic arch with aberrant right subclavian artery, and 1 (1.7%) patient of circumflex aorta with cervical aorta arch. The median ventilator supporting time was 6.0 (0-648) h, and the median hospital stay time was 14.5 (7-104) d. One patient with coarctation of aorta died of severe pulmonary infection during perioperative period, and the others survived without symptoms and reoperation after discharge. The median follow-up time was 7.0 (1-62) months. Conclusion    For children with unexplained dyspnea and dysphagia, or with right aortic arch, preoperative imaging examinations such as computed tomography or magnetic resonance imaging are required to confirm the diagnosis of vascular ring. Surgical correction of congenital vascular ring is safe and reliable, and can effectively relieve symptoms. The mortality rate and reoperation rate are low, and the follow-up results are satisfactory.

Under the guidance and support of national policies in recent years, the community medical system has been developed rapidly, among which primary child healthcare is carried out routinely in community hospitals, greatly alleviating the pressure of specialized pediatric hospitals and departments of pediatrics in secondary and tertiary general hospitals. It is confirmed that nAChRs has important physiological functions in both the nervous system and non-nervous system, and is related to many diseases.nAChRs has also been confirmed to be related to many diseases, so its structure And functional research is very necessary. Therefore, it is necessary to study the structure and function of nAChRs. It is known that the intracellular loop of the nicotinic receptor protein plays a major role in regulating its intracellular metabolism and downstream signal transduction. However, the structure and function of this protein sequence are still unclear. In this paper, the current research status of nicotinic receptor intracellular interaction proteins were reviewed, aiming to further explore the structural and functional characteristics of nicotinic receptor intracellular loops through the information of their interaction proteins and interaction sites, and to provide ideas for clinical targeted therapy.

ABSTRACT

Objective:To study the quality evaluation method of Cyperi Rhizoma processed with four excipients. Method:Ultra-performance liquid chromatography (UPLC) fingerprints of raw products and processed products with four excipients of Cyperi Rhizoma were established, and the changes of chemical components in the fingerprints before and after processing were compared by chemometric analysis. The mobile phase was consisted of methanol (A)-water (B) for gradient elution (0-10 min, 5%-40%A; 10-30 min, 40%-70%A; 30-40 min, 70%A) at a flow rate of 0.3 mL·min<sup>-1</sup>. The injection volume was 3 μL, the column temperature was 35 ℃, and the detection wavelength was 280 nm. The content changes of main index components in Cyperi Rhizoma before and after processing were compared by UPLC. The mobile phase was methanol-water (75∶25) and the detection wavelength was 242 nm. Result:Processing with four excipients had a significant impact on the overall characteristics of chemical components in the fingerprint of Cyperi Rhizoma. A total of 28 characteristic peaks were identified in fingerprints of the raw and processed products. Among them, peaks 1, 2 and 4 were specific peaks of the processed products, peak 5 was characteristic peak of the raw products. Peak 2 was identified as 5-hydroxymethylfurfural, peak 24 as cyperenone and peak 27 as <italic>α</italic>-cyperone. The 5-hydroxymethylfurfural produced by the processing with four excipients came from rice vinegar, rice wine and Maillard reaction of polysaccharides in Cyperi Rhizoma. The results of determination showed that there was no significant difference in the content of cyperenone after processing, but the content of <italic>α</italic>-cyperone decreased significantly. Conclusion:In the process of Cyperi Rhizoma processed with four excipients, there are new components produced by structural transformation, which are accompanied by changes in the content of index components. In this study, the quality of raw and processed products of Cyperi Rhizoma can be rapidly and effectively evaluated from qualitative and quantitative aspects.

ABSTRACT

Objective:To establish the ultra-performance liquid chromatography (UPLC) fingerprints of Artemisiae Argyi Folium and Artemisiae Argyi Folium processed with four excipients, and quantitatively analyze the 8 phenolic acids and flavonoids contained in them, in order to explore the quality evaluation method of Artemisiae Argyi Folium processed with four excipients. Method:UPLC was used with Shim-pack XR-ODS C<sub>18</sub> column (2.0 mm×75 mm, 2.2 µm), mobile phase of acetonitrile (A) -0.2% formic acid aqueous solution (B) for gradient elution (0-1 min, 10%A; 1-2 min, 10%-15%A; 2-17 min, 15%-18%A; 17-24 min, 18%-28%A; 24-36 min, 28%-38%A; 36-41 min, 38%-60%A; 41-45 min, 60%-100%A), detection wavelength of 330 nm and flow rate of 0.2 mL·min<sup>-1</sup>. The UPLC fingerprints of Artemisiae Argyi Folium before and after processing were established, and analyzed by chemometrics. Contents of 5-caffeoylquinic acid, 3-caffeoylquinic acid, 4-caffeoylquinic acid, 3,4-dicaffeoylquinic acid, 3,5-dicaffeoylquinic acid, 4,5-dicaffeoylquinic acid, jaceosidin and epuatilin in the decoction pieces were determined. Result:The fingerprints of Artemisiae Argyi Folium before and after processing were established, and the UPLC characteristic chromatograms of Artemisiae Argyi Folium before and after processing had good consistency, and the similarity was >0.94. Compared with Artemisiae Argyi Folium, the contents of 3-caffeoylquinic acid and 3,4-dicaffeoylquinic acid had no significant change after processing, the contents of jaceosidin and epuatilin decreased after processing, while the contents of 5-caffeoylquinic acid, 4-caffeoylquinic acid and 4,5-dicaffeoylquinic acid increased significantly (<italic>P<</italic>0.01), their average increasing rates were 32.50%, 66.83%, 29.39%, respectively. And content of 3,5-dicaffeoylquinic acid was significantly decreased (<italic>P<</italic>0.01) , and the average reduction rate was 51.25%. Conclusion:The contents of chemical components in Artemisiae Argyi Folium and Artemisiae Argyi Folium processed with four excipients have changed to a certain extent. Among them, 5-caffeoylquinic acid, 3,5-dicaffeoylquinic acid, 4-caffeoylquinic acid and 4,5-dicaffeoylquinic acid can be used as the key indicators for quality evaluation of Artemisiae Argyi Folium before and after processing.

@#Objective    To explore the clinical effect of mitral valvuloplasty on children with Barlow disease combined with moderate to severe or severe mitral regurgitation. Methods    The clinical data of 10 patients with Barlow disease combined with moderate to severe or severe mitral regurgitation in Fuwai Hospital from January 2014 to August 2019 were analyzed retrospectively, including 3 males and 7 females, with a mean age of 8.7±7.9 years. Echocardiography before and during the operation confirmed that the mitral valve leaflets were long and swinging, the valve leaflets and the opposite edge protruded into the left atrium and were higher than the level of the mitral valve rings, the mitral valve rings were dilated, the papillary muscles and tendons were long, and the pathological changes after the operation showed mucoid degenertion of the valve leaflets and tendons, and some fibrous foci hyperplasia. Mitral valve repair included implantation of artificial valve ring, implantation of artificial tendon, posterior leaflets sliding, partial resection of posterior leaflets (excluding sliding), valve leaflets folding, tendon folding, papillary muscle splitting and annular valve contraction (excluding artificial valve ring implantation). The technique of mitral valve repair, early clinical results and follow-up echocardiographic data were analyzed. Results    All the patients successfully completed the mitral valve repair. The mean time of aortic occlusion was 73.2±17.4 min, and cardiopulmonary bypass time was 99.5±19.8 min. At the same time, 4 patients received tricuspid valve repair and 1 funnel chest correction. There was no reoperation in perioperative period. The 1-year and 5-year survival rates were 100.0% and 100.0%, respectively. The incidence of below moderate mitral regurgitation was 90.0% at postoperative 1 year and 72.0% at postoperative 5 years. Conclusion    For the young children who have Barlow disease and mitral regurgitation, considering the characteristics of heavy lesions, small operation space, and the need to meet the growth and development of valve, it is suggested to adopt the surgical techniques different from those of older children, such as valve ring retraction and tendon folding, if necessary, to adopt "edge to edge" suture, which can shorten aortic occlusion time and achieve good early effects, and its long-term effects still need further follow-up observation. Mitral valvuloplasty technique for Barlow disease similar to that of adults can be used in older children, including implantation of artificial valve ring and implantation of artificial tendon, etc.

ABSTRACT

Chimeric antigen receptor T cell (CAR-T) therapy was awarded as the largest research breakthrough in 2017 by the American Society of Clinical Oncology, at present, it is rapidly becoming the most promising new treatment for hematological malignancies. However, this therapy also produces a new challenge: toxic adverse events such as cytokine release syndrome (CRS) and neurotoxicity, partial of them can bring death to the patients. The incidence and severity of the above toxic events in different multi-center trial reports are also different, which may be attributed to the different in the considerably variable assessment and grading of toxicities between clinical trials and across institutions. The ASTCT published at 2018 advanced the consensus grading for cytokine release syndrome and neurologic toxicity associated with immune effector cells, it was focusing on CRS and neurotoxicity associated with immune effector cells. In order to provide reference for the development of relevant work in this field and the formulation of security strategies in our country, the main content of the consensus was summarized briefly.

ABSTRACT

An UPLC-MS/MS method was developed to simultaneously determine complanatoside A and complanatoside B in rat plasma with rutin as the internal standard and applied to examine the effect of salt-processing on pharmacokinetics of these two flavonoid glycosides. The pharmacokinetic parameters were estimated using DAS 3.2.6 and subjected to independent sample t-test with SPSS 23.0. No significant difference in T_(max) of complanatoside B was observed between the raw and processed groups; however, in the processed group, C_(max) and AUC_(0-12 h) of complanatoside B increased obviously(P<0.05), while MRT_(0-12 h) decreased from(3.34±0.44) h to(1.81±0.36) h(P<0.05). C_(max) [(14.72±11.13) μg·L~(-1)] and MRT_(0-24) [(3.93±0.26) h] of complanatoside A in the raw group were statistically different from those [(35.64±21.99) μg·L~(-1),(1.43±0.24) h] in the processed group(P<0.05). As a result, salt-processing can facilitate the in vivo adsorption and accelerate the excretion of complanatoside A and complanatoside B.
